To register as a patient please ask at reception for details or do it online.
Once you have filled in the correct forms and handed them into the surgery, it will take up to 2 weeks to become registered. Please ensure you have adequate supply of your medication from your previous surgery. We will be unable to supply you with any medication until the registration has been processed.
Forms not filled in correctly may effect the time it takes to register so please fill in all parts where applicable.
If you wish to choose to see a particular partner for some or all medical conditions please let the receptionist know and she will note this in your medical records. We will always try to ensure that this is possible but there may be occasions when it is not possible.
