How do I register?
Registering Online
We are using a new online service called Register with a GP surgery: Barnoldswick MED CTR, Barnoldswick that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID, or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one. Please see below for more information.

Registering using paper forms
If you wish to register using paper forms, please clink on the links appropriate to you and print off the forms to complete them. Once you have completed them, you can either hand them in at the surgery or email them to [email protected]. Alternatively, you can call into the surgery to collect a Patient Registration Pack.
Please also complete our new patient questionnaire and EPS nomination form. Failure to do this may result in a delay in your registration. We kindly ask that you also read through the patient information leaflet.

Forms not filled in correctly may affect the time it takes to register so please fill in all parts where applicable.
We may ask to see the following documents from you. You will be contacted by a member of our registration team if this applies to your registration.
•Photo Identification ie. passport or drivers’ licence etc
•Proof of current address ie. utility bill
•Birth certificate for new babies or children with no passport.
Please also make sure you bring with you a list of medication you need if you are on any repeat medication. The registration process may take up to 2 weeks so please ensure you have adequate supply of your medication from your previous surgery. We will be unable to supply you with any medication until the registration has been processed. We need evidence of your medication i.e. last prescription, evidence from previous doctors or tablet boxes.
