We are happy to accept new patients at Knockin Medical Centre. You just need to be permanently resident within our practice catchment area (details under Practice Boundary).
If you wish to register please download and print off the New Registration Form from the link below. Please bring this completed form along with you and one form of photo ID to the practice where our Admin Team will be happy to help you.
If you are on regular medication you will probably require an appointment with one of our GPs before your first prescription can be dispensed.
You can be registered as a temporary patient for up to fourteen days. If you intend to stay in the area for more than fourteen days, and your medical problem has not resolved, your temporary residence status can be extended for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you may be required to register permanently with the practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
The form below needs to be completed and brought in to the surgery.
